Job Overview

As a vital member of the Critical Care Interprofessional team, the Clinical Social Worker will deliver essential psychosocial support to patients and their families.

Key Responsibilities:

  • Conduct thorough social work and psychosocial evaluations on inpatients, utilizing diverse modalities in alignment with professional standards and clinical policies.
  • Coordinate, develop, document, and execute the care plan for hospital discharge, ensuring a safe transition back to community or institutional settings.
  • Engage in team conferences, interdisciplinary educational rounds, and collaborative discussions.
  • Intervene to mitigate high-risk or vulnerable situations.
  • Offer support to patients and families to facilitate adjustment to health-related issues and resolve complex interpersonal challenges.
  • Educate regarding the HCCA and/or Substitute Decisions Act.
  • Provide selected therapeutic interventions, supportive counseling, community outreach, and advocacy.
  • Guide patients on financial, legal matters, workplace injuries, or community referrals.
  • Support visitors requiring hospitalization.
  • Participate in advanced care planning and goals of care discussions through family meetings and mediated conversations.

Qualifications:

Halton Healthcare seeks to employ individuals who are proficient in their profession and embody our mission, vision, and values of accountability, respect, and compassion.

  • BSW or MSW required.
  • Current registration and membership in good standing with OCSWSSW.
  • Experience in providing social work services to patients within an acute care hospital environment.
  • Strong clinical competencies in assessment, counseling, and discharge planning/community integration.
  • Extensive knowledge of community resources and proven ability to collaborate effectively with community partners.
  • Familiarity with the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Healthcare Consent Act.
  • Demonstrated commitment to maintaining an evidence-based practice approach.
  • Previous group experience with this population is preferred.
  • Commitment to ongoing professional development.
  • Ability to document in accordance with college standards.
  • Capacity to function independently and exhibit critical thinking and effective problem-solving skills.
  • Experience in conducting interviews and providing comprehensive psycho-social assessments.
  • Strong understanding of the Public Guardian and Trustee Office and Consent and Capacity Board.
  • Excellent employment and attendance record.

Additional Information:

Hours: Monday to Friday - Day shift and/or evenings (hours subject to change based on operational requirements).

Site: Tri-site.

Salary: Competitive hourly rate.
