Job Summary

We are seeking a highly skilled Social Worker to join our interdisciplinary team at Halton Healthcare. As a Social Worker - Neuro Rehabilitation Specialist, you will play a critical role in assessing, planning, and implementing patient care plans to ensure optimal reintegration into their community setting.

Key Responsibilities
  • Conduct comprehensive social work assessments to identify patient needs and develop personalized care plans.
  • Collaborate with the inter-disciplinary team to address ongoing patient needs and ensure coordination of community-based support services.
  • Provide education and support to patients and their families to facilitate a smooth transition back into the community.
  • Engage patients in addressing socio-emotional health issues and identify their network of supports.
  • Formulate care plans to achieve patient goals and resolve complex issues through conflict resolution and advocacy.
Qualifications
  • Master of Social Work (MSW) degree or Bachelor of Social Work (BSW) degree with relevant experience.
  • Current registration and membership in good standing with the College of Social Workers and Social Service Workers.
  • Minimum 3 years of experience in a hospital or community health organization, preferably in a neurology/stroke setting.
  • Strong communication and interpersonal skills, with the ability to work collaboratively with patients, families, and interprofessional team members.
  • Ability to implement planned social work interventions and address unique health, social, and service challenges for patients and their families.
  • High degree of initiative, self-direction, and ability to multitask in a fast-paced environment.
  • Demonstrated ability to function effectively as a member of the healthcare team.
  • Satisfactory employment and attendance record.
  • Comprehensive knowledge of community resources and research skills to secure new resources.
