Key Responsibilities- Conduct comprehensive social work and psychosocial assessments on inpatients, utilizing a variety of modalities and adhering to professional standards, clinical policies, and practices.
- Develop, document, and implement care plans for hospital discharge, ensuring safe transition to community or institutional settings.
- Participate in team conferences, educational rounds, and interdisciplinary discussions to ensure comprehensive patient care.
- Provide support to patients and families to alleviate high-risk or vulnerable situations.
- Offer education and guidance on healthcare consent and capacity, as well as substitute decision-making.
- Provide selected therapeutic interventions, supportive counseling, community outreach, and advocacy services.
- Offer guidance on finances, legal issues, workplace injuries, and community referrals.
- Support visitors requiring hospitalization and participate in advanced care planning and goals of care discussions.
- Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree required.
- Current registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Experience providing scope of practice social work to patients within an acute care hospital setting.
- Strong clinical skills, including assessment, counseling, and discharge planning/community integration.
- Strong knowledge of community resources and ability to work collaboratively with community partners.
- Familiarity with relevant legislation, including the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Healthcare Consent Act.
- Ability to maintain an evidence-based practice approach and demonstrate commitment to professional development.
- Ability to document according to college standards and function independently, thinking critically and problem-solving effectively.
- Experience in interviewing skills and ability to provide comprehensive and timely psycho-social assessments.
- Strong knowledge of the Public Guardian and Trustee Office and Consent and Capacity Board.
- Excellent employment and attendance record.
