Nurse Care Manager

Job Summary:
The Nurse Care Manager plays a pivotal role in delivering high-quality, client-centered care to individuals and families within their homes and communities. This position is responsible for managing and coordinating care, evaluating health outcomes, educating clients, and advocating for their needs.
About the Role:
This dynamic role requires a skilled nurse who can assess clients' needs, develop personalized care plans, and implement evidence-based nursing practices. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work collaboratively with multidisciplinary teams and stakeholders.
Key Responsibilities:
- Manage and coordinate care for clients with complex needs
- Evaluate health outcomes and make data-driven decisions to improve care
- Provide education and support to clients and families to promote independence and self-care
- Develop and implement care plans that meet clients' unique needs and goals
- Collaborate with healthcare professionals to ensure seamless care coordination
Requirements:
To be successful in this role, you must have:
- A current license as a Registered Nurse in the province of practice
- Certification in Community Health Nursing (CHN) or equivalent experience
- Experience working in community health settings, preferably in a care management role
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
