The Nova Scotia Health Authority is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in the assessment, care planning, and authorization of services for clients referred to Continuing Care.
Key Responsibilities:
- Assess clients' needs and develop personalized care plans to meet their goals and objectives.
- Authorize services and make referrals to other professional and volunteer resources to support community-dwelling clients.
- Work with interprofessional teams to facilitate hospital discharge and return to the community.
- Collaborate with clients, family, caregivers, care providers, and healthcare teams to identify and address unmet needs.
- Reassess clients at routine intervals and provide skilled care management.
About You:
- You have a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
- You are currently registered (or eligible for registration) with the applicable Nova Scotia Professional Regulatory body.
- You have a minimum of three years' experience with the professional regulatory body.
- You have demonstrated knowledge and/or experience in a community or long-term care setting.
- You have knowledge and/or experience in dedicated case management or discharge planning.
- You are competent with computer use, keyboarding, and able to conduct assessments using a laptop.
- You have knowledge of community agencies and resources.
- You possess excellent assessment, interviewing, and negotiation skills.
- You have excellent verbal and written communication skills.
- You are able to work independently and co-operatively in a busy multidisciplinary situation.
- You are able to multi-task, prioritize workload, and manage time efficiently.
- You have a valid driver's license and access to a reliable vehicle.
- Competencies in other languages are preferred, with French being an asset.
