About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in the assessment, care planning, and authorization of services for clients referred to Continuing Care.

Key Responsibilities:

  • Assess clients' needs and develop personalized care plans to meet their goals and objectives.
  • Authorize services and make referrals to other professional and volunteer resources to support community-dwelling clients.
  • Work with interprofessional teams to facilitate hospital discharge and return to the community.
  • Collaborate with clients, family, caregivers, care providers, and healthcare teams to identify and address unmet needs.
  • Reassess clients at routine intervals and provide skilled care management.

About You:

  • You have a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
  • You are currently registered (or eligible for registration) with the applicable Nova Scotia Professional Regulatory body.
  • You have a minimum of three years' experience with the professional regulatory body.
  • You have demonstrated knowledge and/or experience in a community or long-term care setting.
  • You have knowledge and/or experience in dedicated case management or discharge planning.
  • You are competent with computer use, keyboarding, and able to conduct assessments using a laptop.
  • You have knowledge of community agencies and resources.
  • You possess excellent assessment, interviewing, and negotiation skills.
  • You have excellent verbal and written communication skills.
  • You are able to work independently and co-operatively in a busy multidisciplinary situation.
  • You are able to multi-task, prioritize workload, and manage time efficiently.
  • You have a valid driver's license and access to a reliable vehicle.
  • Competencies in other languages are preferred, with French being an asset.
