Job Overview

This full-time contract position offers the opportunity to work with a dynamic team of professionals dedicated to transforming lives. As an Intensive Case Management Worker, you'll play a crucial role in helping clients navigate the complexities of homelessness, connecting them to vital resources and services.

Responsibilities

  1. Deliver high-quality case management services to clients, including accessing housing options, completing documentation, and connecting clients to essential services.
  2. Collaborate with colleagues to develop comprehensive client case plans, ensuring seamless transitions and continuity of care.
  3. Maintain accurate and detailed records, using case management software to track progress and identify areas for improvement.
  4. Prioritize conflicts and crises, utilizing effective negotiation and conflict resolution techniques to ensure positive outcomes for clients.

Essential Skills

  • Proven experience in crisis intervention, conflict resolution, and negotiation.
  • Demonstrated ability to work effectively in a fast-paced environment, prioritizing tasks and managing competing demands.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, colleagues, and external stakeholders.
  • Familiarity with relevant policies, procedures, and legislation impacting homelessness and social services.
