Job Title: Community Health Nurse
Bayshore Home Care Solutions is seeking a passionate and caring Registered Nurse (RN) for community nursing roles. As a Community Health Nurse, you will provide high-quality nursing care to clients in their homes, promoting independence and well-being.
Job Responsibilities:
- Provide comprehensive nursing care to clients in their homes, including wound care, intravenous therapy, and health teaching.
- Assess clients' needs and develop individualized care plans to meet their unique requirements.
- Collaborate with clients, families, and healthcare teams to ensure seamless care coordination.
- Participate in ongoing education and training to enhance your skills and knowledge.
Requirements:
- Current registration with the College of Nurses of Ontario.
- Valid driver's license and reliable vehicle.
- Up-to-date immunization records and clear criminal background check.
- Current CPR certification.
