Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our community nursing team. As a Community Nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, and retirement homes, providing exceptional nursing services to patients and their families.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide nursing services in a variety of settings, including in-home, clinics, and retirement homes
  • Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care coordination
  • Perform other clinical duties as required
Requirements
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • Valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
