The Role:

We are seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will work independently or in cooperation with the Home and Community Care Support Services (HCCSS) to develop and maintain effective care plans for patients in their homes, clinics, retirement homes, or schools.

Key Responsibilities:

  • Develop and implement individualized care plans that meet the unique needs of patients
  • Provide exceptional nursing services to patients in their homes, clinics, retirement homes, or schools
  • Communicate effectively with patients, family members, and other healthcare professionals to ensure seamless care coordination
  • Perform other clinical duties as required

Requirements:

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of patients
  • Exceptional interpersonal skills
  • A valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record, including 2-step TB test
  • Vulnerable sector check and clear background check
