Job Title: SCOPE Navigator, Community Integration
The SCOPE Navigator plays a vital role in leading the system navigation function of the SCOPE program at North York General Hospital. This position collaborates with the interprofessional team and interdisciplinary supports in the North York community to provide leadership that helps planning, facilitate change, evaluate, and coordinate with all stakeholders.
This role serves as a single point of access for primary care providers to access appropriate resources, triaging, assessing, planning, and communicating with the interdisciplinary team. The SCOPE Navigator will be the leader in developing individualized care plans with internal and external stakeholders to facilitate seamless care for primary care providers and their patients.
Key Responsibilities:
- Serve as a centralized point of access to connect primary care providers with appropriate community, clinical, and specialized supports for complex patients across the healthcare continuum.
- Use assessment skills to collect relevant patient information required for supporting care coordination for the patient.
- Assess the appropriateness of the call for SCOPE by using critical thinking skills.
- Triage calls appropriately by understanding the difference between cases that require General Internal Medicine support, Medical Imaging support, Palliative Care support, Mental Health and Addiction support, and other services.
- Navigate different clinics and departments in and out of the hospital to support primary care providers with managing medical, functional, cognitive, and psychosocial conditions.
- Enhance workflow and communication pathways between hospital, primary care, and community care by identifying gaps in care and promoting new seamless pathways that optimize patient care.
Qualifications:
- Regulated Allied health professional in good standing with the respected registered college.
- Minimum 3 years of experience in the acute care or community setting.
- Computer proficient and able to work remotely.
- Relationship management with primary care providers, the interdisciplinary team, specialists, community support service providers, North York-based clinics, and their administrative assistants.
- Strong knowledge of hospital/community resources.
- Ability to problem solve and critically think through complex cases requiring multidisciplinary coordination.
- 3 years of experience in caring for persons living with chronic disease and mental health disorders preferred.
- Bilingualism in French/English is an asset.

This is a Full-Time Temporary Position of 18 months with 8-hour shifts, Days shifts in the Community Integration Department.
