Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Sunrise Senior Living. As a key member of our healthcare team, you will be responsible for providing high-quality resident care and services, promoting a culture of wellness and dignity, and collaborating with our interdisciplinary team to deliver exceptional care.

Responsibilities
  • Conduct thorough assessments of residents' needs and develop individualized care plans to ensure their physical, emotional, and social well-being.
  • Administer medications and treatments as prescribed by physicians, and document all care provided in resident records.
  • Collaborate with the Resident Care Director and other healthcare professionals to coordinate care and services, and ensure seamless transitions between shifts.
  • Provide education and support to residents and their families on health-related topics, and promote healthy lifestyle choices.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote resident-centered care.
  • Maintain accurate and up-to-date records of resident care, including medication administration, vital signs, and other relevant information.
  • Communicate effectively with residents, families, and healthcare professionals to ensure that care is coordinated and resident needs are met.
Qualifications
  • Graduate of an accredited Licensed Practical Nurse (LPN) program.
  • Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one year of experience in a healthcare setting, preferably in long-term care or assisted living.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a focus on providing high-quality resident care and services.
  • Basic computer skills and proficiency in electronic medical records (EMRs).
