About the Role

CarePartners is seeking a skilled Team Coordinator to join our team. As a Team Coordinator, you will be the bridge between patients, families, and staff, ensuring effective customer service and communication.

Key Responsibilities

  • Develop and coordinate service schedules while considering relevant factors.
  • Inform workers of new assignments and changes to their schedules.
  • Enter relevant information into the automated information system/database.
  • Produce schedules and reports on a regular basis.
  • Receive and manage inbound telephone calls and email inquiries related to service.
  • Perform other administrative duties.

Requirements

  • A strong desire and commitment to making a difference in the lives of patients.
  • Post-secondary education is preferred and one (1) year of job-related experience is required.
  • Exceptional interpersonal and customer service skills.
  • Ability to work both independently and as part of a team.
  • Strong proficiency in all aspects of Microsoft Office.
  • A strong attention to detail and the ability to learn new systems and databases.
  • Effective verbal and written communication skills.
