Key Responsibilities:
- Supervise and support a team of healthcare professionals to deliver high-quality care to clients in their homes.
- Develop and implement care plans that meet the unique needs of each client.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Provide training and mentorship to team members to enhance their skills and knowledge.
- Conduct regular home visits to assess client needs and provide support.
Requirements:
- Current registration with the College of Nurses of Ontario.
- Minimum 10 years of experience in community nursing, with a focus on leadership and team management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
