Job Summary
We are seeking an experienced Care and Support Specialist to join our vibrant team. As a key member of our team, you will provide care and support to individuals living in staffed homes within their community. Your role will involve empowering individuals to make informed decisions about their lives, respecting their rights and choices, and advocating as needed.
Key Responsibilities
The successful candidate will be responsible for ensuring the supported persons' rights and choices are respected and advocating as needed. This includes interacting effectively with people we support, empowering them to be full citizens of their community, and participating in the development, implementation, and documentation of Personal Service Plans. Additionally, you will foster positive relationships with family members, professionals, coworkers, and community members, assist with activities of daily living and medication administration, and record daily activity and personal health information (PHI) of the people we support on our case management database.
Required Skills and Qualifications
To be successful in this role, you will require a Grade 12 education, a Community Support Worker Diploma or equivalent, and a minimum of two years of experience in community social services/healthcare. You must also possess current Emergency First Aid/CPR and Food Safe certificates, previous behavior management course/training, and ability to effectively communicate verbally and in writing with team members, professionals, families, and community members. A restricted class 4 driver's license, satisfactory driver's abstract, criminal record clearance for children and vulnerable adults, proof of COVID-19 vaccination status, and TB test are also required. Finally, you should have moderate computer literacy and eligibility to work in Canada.
