Job Summary

The Personal Support Worker provides personalized care and support services to clients and families within the framework of VON policies, procedures, and standards. This role involves collaborating with healthcare professionals to meet the needs of clients and promote their independence and well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure their needs are met.
  • Develop and implement personalized care plans in collaboration with the Personal Support Program Manager.
  • Provide personal care services, including light housekeeping, meal preparation, and assistance with daily living activities.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Monitor and maintain household safety, and report any concerns to the supervisor or Personal Support Program Manager.
  • Provide respite care and support to clients' families as needed.
  • Organize daily activities, prioritize tasks, and maintain accurate records to ensure efficient service delivery.
  • Assess clients' communication patterns and abilities, and communicate effectively with healthcare professionals to ensure comprehensive care.
  • Function in accordance with Occupational Health and Safety legislation, VON policies, and procedures to promote client safety.
Requirements
  • High school diploma or equivalent.
  • CCA Certification (Nova Scotia) or equivalent.
  • Personal Support Worker program or Health Care Aide certification.
  • Standard First Aid Certificate or CPR level C Certificate (if applicable).
  • Experience working with elderly or ill individuals or children.
  • Strong communication and assessment skills.
  • Awareness of community resources.
  • Ability to work effectively in a multidisciplinary team or independently.
