Job Summary

The Personal Support Worker provides personalized support and homemaking services to clients and families within the framework of VON policies, procedures, and standards.

Key Responsibilities:
  • Conduct thorough assessments of clients' and families' needs to determine the required level of support.
  • Develop and implement personalized care plans in collaboration with the healthcare team.
  • Provide personal care services, including assistance with daily living activities, mobility, and hygiene.
  • Perform light housekeeping and meal preparation tasks to maintain a safe and healthy environment.
  • Monitor and maintain household safety, including the management of medications and medical equipment.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Collaborate with the healthcare team to address any changes in clients' or families' needs or circumstances.
  • Document all interactions, assessments, and care plans in accordance with VON's policies and procedures.
Requirements:
  • High school diploma or equivalent.
  • CCA Certification (Nova Scotia) or equivalent.
  • Personal Support Worker (PSW) or Home Support Worker (HSW) certification.
  • Standard First Aid Certificate or CPR level C Certificate.
  • Experience working with elderly or ill individuals or children.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
Working Conditions:

The Personal Support Worker will work in a variety of settings, including clients' homes, community centers, and VON offices. The role requires flexibility and adaptability to meet the changing needs of clients and families.
