Job Title: Registered Nurse

Job Summary:

VON Canada is seeking a skilled Registered Nurse to join our team in Ontario. As a Registered Nurse, you will provide high-quality, client-centered care to individuals and families in their homes and communities.

Key Responsibilities:

  • Manage and coordinate care, evaluating health outcomes and educating clients and families to meet health goals.
  • Provide all levels of nursing care, from stable clients with predictable outcomes to those with acute, complex, and rapidly changing health needs.
  • Develop and implement care plans, incorporating discharge planning as needed, and ensuring services meet client needs.
  • Collaborate with clients, families, and caregivers to promote independence and self-care.
  • Participate in quality improvement initiatives and contribute to the development of nursing best practices.

Requirements:

  • Licensure as a Registered Nurse in the province of practice.
  • Canadian Nurses Association certification in Community Health Nursing is an asset.
  • Experience in community health and clinical practice is an asset.
