Job DescriptionThe Social Services Manager will be responsible for providing assessment, treatment planning, counseling, and case management services to individuals and families in need. This role will involve working collaboratively with clients, families, and healthcare teams to identify and address social and emotional needs.
Key Responsibilities- Conduct comprehensive assessments to identify client needs and develop individualized treatment plans.
- Provide counseling and case management services to clients and families, including crisis intervention and support.
- Collaborate with healthcare teams to coordinate care and services, ensuring seamless transitions and effective communication.
- Develop and maintain relationships with community agencies and resources to ensure access to support services.
- Provide education and support to clients and families, promoting health and wellness.
- Document client progress and outcomes, ensuring accurate and timely reporting.
- Master's degree in Social Work or related field.
- Minimum 2 years of experience in social services, preferably in a healthcare setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of community resources and services.
- Strong assessment and case management skills.
- Experience working with diverse populations and cultures.
- Knowledge of evidence-based practices and research.
- Skills in crisis intervention and conflict resolution.
- A competitive salary of $83,000 - $110,000 per year, depending on experience.
