About the Role:

We are seeking a highly skilled and dedicated Registered Nurse to join our Transitions team at SE Health. As a Registered Nurse - Home and Community Care Specialist, you will have the opportunity to work with a diverse client base, providing comprehensive care in an interdisciplinary team environment.

Key Responsibilities:

  • Provide continuous and comprehensive support to clients in the community
  • Lead the care planning process and work with other disciplines to complete and implement client care plans
  • Facilitate regular interdisciplinary team communication and lead discharge and transition care planning discussions
  • Collaborate and connect clients with community support services and support clients, families, and caregivers in developing strong self-care strategies

Requirements:

  • Registered in good standing with the College of Nurses of Ontario
  • Bachelor's Degree in nursing preferred
  • Canadian Nurses Association (CNA) community nursing certification is an asset
  • Minimum of two years of nursing experience
  • Experience in home/community care, acute care, chronic disease management, health promotion, palliative care, and mental health
