Job Title: Full-Time Community Service Worker

We are seeking a highly motivated and compassionate Full-Time Community Service Worker to join our team at Thrive Group Canada. As a Community Service Worker, you will play a vital role in supporting individuals with disabilities to live independently in their own homes or one of our independent living facilities.

About the Role:

  • Provide person-centered support to clients to maximize their independence and enhance their quality of life.
  • Work collaboratively with clients, families, and other stakeholders to develop and implement individualized support plans.
  • Assist clients with daily living activities, such as personal care, meal preparation, and household management.
  • Provide emotional support and advocacy to clients to help them navigate the healthcare system and access community resources.
  • Participate in quality improvement initiatives to enhance the delivery of care and services.

Requirements:

  • University degree in Health or Social Sciences field, or College Diploma with three (3) years' work experience in the Health and/or Social Sciences field.
  • Professional designation is an asset.
  • Experience working with seniors and extensive knowledge of the local healthcare system and community resources.
  • Excellent interpersonal, oral and written communication skills.
  • Proven ability to work as a team member and demonstrated ability to make sound judgment decisions independently.
