Job Summary

We are seeking a highly skilled and experienced Client Care Coordinator to join our team at Fraser Health. As a key member of our interprofessional team, you will play a vital role in providing clinical leadership, supervision, and coordination of client care services.

Key Responsibilities
  • Provide clinical leadership and guidance to staff, ensuring high-quality patient care and adherence to standards of practice.
  • Supervise and provide work direction to staff, coordinating client assignments and adjusting work activities to meet the needs of the unit.
  • Coordinate staffing coverage, ensuring adequate staffing levels to meet client care needs.
  • Assist staff in providing quality care, providing information and consultation on specialized theory and practice.
  • Participate in quality improvement and risk management activities, evaluating the effectiveness of clinical practices and making recommendations for improvement.
  • Collaborate with interprofessional team members and other healthcare professionals to identify and resolve client and family issues.
  • Provide consultation and/or screening of clients in special circumstances, and provide program information to clients, their families, and the public.
  • Identify and respond to client care issues, program deficiencies, and emergencies, recommending appropriate action.
  • Set measurable personal performance goals and objectives, incorporating the vision and values of Fraser Health.
Requirements
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three years of recent related clinical experience in the designated clinical area, including one year experience in a supervisory role.
  • Completion of substance use certificate program as required.
  • Successful completion of St. John's Ambulance Safety Orientated First Aid Certificate and current Cardiopulmonary Resuscitation Certificate (CPR) level C or equivalent.
Skills and Abilities
  • Clinical proficiency/expertise in the designated clinical area.
  • Knowledge of community resources and health-related support services.
  • Ability to supervise and provide leadership and work direction.
  • Ability to coordinate client assignments, in collaboration with staff.
  • Ability to monitor staff workload and patient factors and adjust client assignments accordingly.
  • Ability to establish priorities for client care, patient flow/discharge planning, and overall daily operations of the designated unit(s).
  • Ability to monitor the quality of client care and ensure maintenance of standards for nursing practice and nursing support staff.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to work independently and in an organized and self-directed manner.
  • Ability to problem-solve and effectively deal with conflict situations.
