Key Responsibilities
- Participate in scheduled visits to assess and address the needs of patients in their homes
- Provide safe and effective nursing care, using appropriate assessment tools and techniques
- Develop and implement individualized care plans, in collaboration with patients and their families
- Engage patients in their care, using a collaborative approach to build trust and rapport
- Maintain accurate and up-to-date patient records, reflecting patient encounters and care plans
Requirements
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing
- Valid driver's license and access to a reliable vehicle
- Experience in delivering community care, an asset
- Reliability, flexibility, and a willingness to travel to patient residences
- Passion for making a difference in the community
