Palliative Care Nurse

We are seeking a compassionate and skilled Palliative Care Nurse to join our team at ParaMed Home Health Care. As a Palliative Care Nurse, you will have the opportunity to make a meaningful difference in the lives of patients and their families.
About the RoleThis is a full-time opportunity for a Registered Practical Nurse (RPN) with experience in palliative care. You will be working in a community setting, providing care to patients in their homes. The ideal candidate will have a strong commitment to delivering high-quality patient care and a passion for working in a dynamic and supportive team environment.
Key Responsibilities- Provide palliative care to patients in their homes, working collaboratively with other healthcare professionals to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop and implement plans to address these needs.
- Administer medications and treatments as prescribed, and provide education to patients and their families on self-care and disease management.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Document patient care and progress in a timely and accurate manner.
- Current registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).
- Experience in palliative care, with a strong understanding of the principles and practices of palliative care.
- Valid driver's license and access to a reliable vehicle.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
