Intensive Case Manager

Island Health is seeking a highly skilled and compassionate professional to join our team as an Intensive Case Manager. This is a challenging and rewarding role that requires a strong understanding of mental health and substance use issues, as well as excellent communication and interpersonal skills.
Key Responsibilities- Provide intensive case management services to clients with complex mental health and substance use needs.
- Develop and implement individualized treatment plans that address the client's physical, emotional, and social needs.
- Work collaboratively with a multidisciplinary team to provide comprehensive care and support to clients.
- Conduct thorough assessments and evaluations to identify client needs and develop effective treatment strategies.
- Provide education and support to clients and their families on mental health and substance use issues.
- Develop and maintain relationships with community resources and services to ensure seamless care coordination.
- Participate in regular team meetings and contribute to the development of treatment plans and goals.
- Baccalaureate degree in a health-related discipline from an approved post-secondary institution.
- Recognized post-basic education related to mental illness and/or substance use.
- Two years of recent related experience supporting clients with substance use and mental health issues in a community setting.
- Strong knowledge of harm reduction philosophy and practice, as well as HIV, HCV, and related issues.
- Experience working with persons living with substance use and mental health concerns.
- Experience in engagement with persons who have severe substance use problems and do not readily engage in healthcare services.
- Understanding and application of substance misuse assessments, treatment methodologies, intervention strategies, the change process, and community dynamics.
- Knowledge of applicable legislation, such as Mental Health and Child Protection legislation.
- Knowledge of available programs and services, their interrelationships, and their function in delivering services in the community.
- Demonstrated competence in the treatment of addiction and mental illness congruent with Island Health's framework and standards within a rehabilitation/recovery, harm-reduction focused model.
- Demonstrated ability to practice from a harm reduction approach.
- Demonstrated ability to build relationships and nurture an environment of quality, trauma-informed, and culturally safe care.
- Effective communication and interpersonal skills.
- Organizational, problem-solving, and teaching skills.
- Judgment, good observation, and client assessment skills, tact, and empathy in working with addiction clients.
- Ability to work collaboratively as part of an interdisciplinary team.
- Promote positive change and independence.
- Prioritize and organize workload.
- Ability to carry out the duties of the position.
- Operate a personal computer with a variety of Windows-based software.
- Foster good public relations.
- Operate related equipment.
