Job Summary

Island Health is seeking a highly skilled and compassionate professional to join our team as an Intensive Case Manager. This role requires a strong understanding of mental health and substance use issues, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Provide intensive case management services to clients with complex mental health and substance use needs.
  • Develop and implement individualized treatment plans in collaboration with clients, families, and healthcare providers.
  • Conduct thorough assessments and provide ongoing support to clients, including crisis intervention and stabilization.
  • Build and maintain relationships with community resources and services to ensure seamless care coordination.
  • Collaborate with interdisciplinary teams to provide comprehensive care and support.
  • Document client progress and outcomes in accordance with Island Health's standards and regulations.
Requirements
  • Baccalaureate degree in a health-related discipline from an approved post-secondary institution.
  • Recognized post-basic education related to mental illness and/or substance use.
  • Two years of recent related experience supporting clients with substance use and mental health issues in a community setting.
  • Strong knowledge of harm reduction philosophy and practice, as well as HIV, HCV, and related issues.
  • Experience working with persons living with substance use and mental health concerns.
  • Valid BC Driver's License.
Preferred Qualifications
  • Demonstrated competence in the treatment of addiction and mental illness congruent with Island Health's framework and standards within a rehabilitation/recovery, harm-reduction focused model.
  • Demonstrated ability to practice from a harm reduction approach.
  • Demonstrated ability to build relationships and nurture an environment of quality, trauma-informed, and culturally safe care.
  • Effective communication and interpersonal skills.
  • Organizational, problem-solving, and teaching skills.
  • Judgment, good observation, and client assessment skills, tact, and empathy in working with addiction clients.
