At Retirement Concepts, we are seeking a skilled Licensed Practical Nurse to join our care team as a leader and coordinator. This role is ideal for a dedicated professional who is passionate about delivering high-quality, resident-centered care.

About the Role

The LPN will report to the Director of Care and be responsible for coordinating care delivery, performing duties, and monitoring departmental activities in accordance with their scope of practice, established standards, regulations, policies, and procedures.

Key Responsibilities

  • Assess residents' needs and implement individualized care plans.
  • Liaise with physicians, community agencies, families, visitors, volunteers, and other disciplines to ensure quality care is delivered.
  • Administer prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives.
  • Monitor and report changes in physical and psychological conditions of residents.
  • Participate in Interdisciplinary Care Conferences and care plan reviews.

Requirements

  • Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in a long-term care facility is an asset.
