Job Summary
We are seeking an experienced Registered Nurse or Registered Psychiatric Nurse to join our team as a Community Mental Health Nurse Specialist. This role will involve providing treatment services to individuals with mental health and/or concurrent disorders. The successful candidate will work collaboratively with our interprofessional care team to develop and implement comprehensive treatment plans tailored to each client's needs.
Responsibilities
* Coordinate care for clients by working closely with our interprofessional care team to perform intake functions, conduct mental health assessments, develop and implement individualized treatment plans, administer prescribed medications, and monitor clients' mental state and response to treatment.
* Plan and provide direct nursing care to clients in compliance with standards of practice, including reviewing client history, conducting physical and psychosocial assessments, assessing vital signs, developing a care plan specific to each client, and collaborating with other healthcare team members to discuss and review the care plan.
* Establish and maintain therapeutic relationships based on respect with clients and their informal support networks by engaging stakeholders in treatment planning and evaluation, providing support and follow-up planning, promoting self-care, encouraging informed decision-making, and empowering clients to improve their quality of life. Provide education to clients and their families about management and treatment of psychiatric conditions, concurrent substance use issues, symptoms, and integration into their lifestyle.
* Treat services include crisis intervention, individual counseling, and education according to various theoretical models to clients and their informal support networks as needed. Assess situations, elicit concerns, explore emotional responses to illness, identify goals and expectations, review client and family learning needs, assist with understanding of illness, teach coping strategies, and consult with colleagues when necessary.
* Participate in case conferences and meetings with clients, family, service providers, and interprofessional team members to identify client mental health issues, concerns regarding services, coordination, emergent issues, and ongoing follow-up care to ensure a coordinated approach to treatment. Act as a client advocate to protect and promote clients' rights to autonomy, respect, privacy, dignity, and access to information.
* Consult and collaborate with interprofessional care team members, including physicians, primary care providers, team coordinators, clinical nurse educators, and other healthcare professionals/providers, to identify and resolve issues, adjust care plans as needed, and address changes in client condition.
* Monitor extended leave as required under the Mental Health Act by reviewing conditions, determining if clients are following conditions, tracking renewal certification due dates, and ensuring continuity of appropriate treatment.
* Assist clients in accessing services such as medical appointments, hospitals, and social programs by providing healthcare information, contacting relevant programs/services, assisting with forms, transportation issues, and daily living tasks.
* Document assessments, care plans, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
* Maintain a safe environment for clients, visitors, and employees by adhering to established safety and emergency practices and procedures and reporting any risks and anomalies to the Team Coordinator.
* Identify own learning needs/goals, review these with the Team Coordinator, and engage in various activities to meet these needs/goals.
* Participate in research and quality improvement activities by identifying client care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures, or protocols.
* May be required to plan, coordinate, and deliver educational sessions to clients and their family members, other service providers, and community agencies regarding mental health and substance use services. Facilitate group sessions using psychodynamic, cognitive/behavioral, interpersonal, and supportive interventions in consultation with interprofessional team members. Provide outreach nursing care to clients in their home setting, primary care setting, or alternative settings, including individuals who are homeless or at risk of homelessness, performing nursing functions such as assessing substance use dependence, evaluating physical and mental status, monitoring withdrawal symptoms, and formulating a plan of care. Transport and accompany clients to physician appointments for assessment to determine suitability for program or designated safe bed for withdrawal management support. Include working with residential facilities or community agencies/homes to conduct client assessments, facilitate placement, develop individualized care/service plans, monitor progress, and ensure referrals and discharges as needed. As assigned, demonstrate and articulate the responsibility of the authorized RN/RPN prescriber to the interprofessional team and when interacting with clients. Identify when client needs exceed RN/RPN scope of practice.
Qualifications
* Education, Training & Experience
Graduation from an approved school of nursing or psychiatric nursing. Two (2) years of recent experience providing treatment services to individuals with mental health and/or concurrent disorders or an equivalent combination of education, training, and experience. May require completion of a substance use certificate program. May require eligibility for completion of BC Provincial Opiate Addiction Treatment Support Program course and approved related preceptorship.
* Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage.
* Skills and Abilities
* Ability to complete initial and ongoing mental health and substance use client assessment and provide nursing care through appropriate/prescribed technical, therapeutic, safety type interventions including crisis intervention.
* Knowledge of mental illness and substance use treatment
* Knowledge of psychopharmacology (indications and side-effects).
* Counselling and group facilitation skills.
* Knowledge of legislation and government policies and procedures that relate to mental health and substance use practices.
* Ability to communicate effectively verbally and in writing with clients, families/care givers, the public, medical staff, and members of the interprofessional care team.
* Ability to promote person-centered care that demonstrates empathy for clients/families/care givers, sensitivity to diverse cultures and preferences, including advocacy for special populations.
* Ability to educate clients and families/care givers about topics essential to their health care and well-being.
* Ability to organize and prioritize work.
* Ability to collaborate with and promote cooperation among members of the interprofessional care team.
* Ability to use factual information and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
* Ability to operate related equipment, including relevant computer applications.
* Physical ability to perform the duties of the position.
Estimated Salary
$45.36 - $62.37 per hour
