Job Title: Unit Coordinator

Job Summary:

The Unit Coordinator plays a vital role in the Emergency Department at Lakeridge Health, ensuring seamless patient flow and discharge processes. This position requires strong organizational and communication skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Engage in daily discharge rounds for admitted patients, maintaining thorough output and identifying opportunities for improvement.
  • Facilitate appropriate discharge planning and coordinate transfers to other sites as required.
  • Work closely with the inter-disciplinary team, including nurses, therapists, and social workers, to ensure comprehensive patient care.
  • Develop and maintain data accountability, tracking patient progress and outcomes.
  • Act as a steward for developing and facilitating family-centered care approaches.

Qualifications:

  • Registration with the College of Nurses of Ontario (in good standing).
  • Current BCLS and ACLS with Medical Directives.
  • Recent and relevant Charge Nurse experience (within the last 2-3 years).
  • Significant and recent experience in an Acute Medical Unit (within the last year).
  • Recent experience in Emergency/Critical Care/Intensive Care nursing (preferred).
  • Proven experience with staff scheduling, patient flow, and patient assignments.
  • B.Sc.N (preferred).
  • Demonstrated leadership, teaching, and mentorship skills.
  • Effective organizational, planning, prioritization, and time management skills.
  • Demonstrated ability to deal with multiple competing demands and maintain composure in stressful situations.
  • Proven ability to work well independently and demonstrate well-developed critical thinking skills.
  • Ability to collaborate with Lakeridge Health Patient Flow, inpatient clinical program interdisciplinary teams, and community partners for quality-based patient outcomes.

  • The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).
