About the Role

CarePartners is seeking a skilled and compassionate Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Nurse. As a Community Nurse, you will have the opportunity to provide exceptional nursing services to patients in their homes, clinics, and other community settings.

Key Responsibilities
  • Develop and maintain effective care plans for patients receiving community health services
  • Provide nursing services to patients in various settings, including in-home, clinic, and retirement home settings
  • Communicate and collaborate with patients, family members, and other healthcare professionals to ensure seamless care
  • Perform other clinical duties as required

Requirements
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • Valid driver's license and vehicle
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
