Job Summary
We are seeking a Registered Practical Nurse to join our team in Brampton. As a member of our community care team, you will provide high-quality nursing care to patients in their homes, working collaboratively with other healthcare professionals to achieve successful outcomes.
Key Responsibilities
- Provide patient-focused, safe, and holistic nursing care to patients in their homes
- Work collaboratively with other members of the healthcare team to achieve successful outcomes
- Continually assess, anticipate, and respond to the changing needs of patients
- Travel within your region to provide care to patients in their homes
Requirements
- Minimum two years of nursing experience, preferably in a community setting
- Registered and in good standing with the College of Nurses of Ontario (CNO)
- Ability to work independently in the community
- Proficiency in English, both verbal and written, required (additional languages an asset)
- Valid Ontario driver's license and reliable vehicle
- Experience with infusion therapy, mental health, palliative care, and wound care
- Excellent patient assessment and teaching skills
