Role Overview



We are currently seeking a Registered Practical Nurse (RPN) to join our Community Nursing Team in Simcoe and Delhi areas of Norfolk County. As an RPN, you will play a vital role in providing holistic care to our clients, including palliative, pediatric, wound care, intravenous therapy, and health teaching chronic disease prevention.



Responsibilities




  • Provide high-quality nursing care to clients in their homes, focusing on palliative, pediatric, wound care, intravenous therapy, and health teaching chronic disease prevention.
  • Assess clients' needs and develop personalized care plans in collaboration with healthcare teams.
  • Conduct regular home visits to monitor clients' progress, provide support, and make necessary adjustments to care plans.
  • Maintain accurate records of client interactions, including medication administration, treatments, and other relevant information.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Foster strong relationships with clients and their families, promoting trust, empathy, and understanding.


Requirements



To be successful in this role, you must have:




  • A diploma in Registered Practical Nursing from an accredited School of Nursing.
  • A valid registration with the College of Nurses of Ontario.
  • A current CPR certification and updated immunization records.
  • A reliable vehicle and a valid driver's license.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
