Key Responsibilities
As a Community Nurse, your key responsibilities will include:
Developing and implementing effective care plans in collaboration with patients, families, and healthcare teams.
Providing comprehensive nursing care, including wound care, medication management, and education.
Communicating effectively with patients, families, and healthcare teams through various means, including phone, email, and in-person visits.
Maintaining accurate records and reports, including patient charts and medication administration records.
Participating in ongoing education and training to ensure up-to-date knowledge and skills.
Requirements
To be successful in this role, you will need:
A valid RN or RPN registration with the College of Nurses of Ontario.
A strong commitment to delivering high-quality patient care and services.
Excellent communication and interpersonal skills.
A valid driver's license and reliable vehicle for travel.
Current immunization records and a clear background check.
