Job Title: Registered Practical Nurse - Community Care

About the Role:

Bayshore HealthCare is seeking a passionate and caring Registered Practical Nurse (RPN) to join our team in the community nursing roles. As an RPN, you will provide high-quality nursing care to clients in their homes, with a focus on visit nursing and shift clients in the Delhi area of Norfolk County.

Key Responsibilities:

  • Provide nursing care to clients in their homes, including palliative, pediatric, wound care, intravenous therapy, and health teaching chronic disease prevention.
  • Work collaboratively with clients, families, and healthcare teams to develop and implement individualized care plans.
  • Teach, delegate tasks, and functions to different levels of workers and client family members.
  • Participate in comprehensive training, mentorship, and preceptorship programs to increase skills and knowledge.

Requirements:

  • Registered Practical Nurse (RPN) with a valid license in Ontario
  • Current CPR certification
  • Valid driver's license and reliable vehicle
  • Updated immunization records and clear criminal background check
  • Experience with trach, vent considered an asset
