Rehabilitation Support Worker for Persons with Acquired Brain Injury

We are seeking a highly skilled and compassionate Rehabilitation Support Worker to join our team at Brain Injury Services Muskoka Simcoe. As a key member of our interdisciplinary health team, you will play a vital role in supporting individuals with acquired brain injury to achieve their goals and improve their quality of life.
Key Responsibilities- Case Management: Develop and implement individualized plans to support clients in achieving their goals, including assessing needs, identifying resources, and coordinating services.
- Life Skills Training: Provide training and support to clients in areas such as daily living skills, cognitive rehabilitation, and behavioral management.
- Group Facilitation: Lead and facilitate groups to promote socialization, education, and skill-building among clients.
- Collaboration and Communication: Work collaboratively with healthcare professionals, family members, and other stakeholders to ensure comprehensive and coordinated care.
- Documentation and Reporting: Maintain accurate and timely records of client progress, including assessment reports, treatment plans, and outcome evaluations.
- Education: College diploma or equivalent in a related field, such as rehabilitation, social work, or psychology.
- Experience: Previous experience working with individuals with acquired brain injury, preferably in a rehabilitation setting.
- Certifications: CPR, First Aid, and Non-Violent Crisis Intervention (NVCI) certifications.
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
