Rehabilitation Support Worker for Persons with Acquired Brain Injury

We are seeking a skilled Rehabilitation Support Worker to join our team at Brain Injury Services Muskoka Simcoe. The successful candidate will provide support and assistance to individuals with acquired brain injury, helping them to achieve their goals and improve their quality of life.
Key Responsibilities- Prepare reports and case histories to document client progress and needs
- Lead and instruct groups to promote socialization and skill-building
- Implement life skills workshops to enhance client independence
- Develop and implement counseling and intervention programs to support client goal-setting and achievement
- Implement behavior management programs to promote positive behavior change
- Maintain program statistics to inform evaluation and research
- Identify additional or alternative services and provide referrals as needed
- Interview clients to assess their needs and develop individualized plans
- Prepare assessment, progress, and follow-up reports to support client care
- Collaborate with interdisciplinary health teams to ensure comprehensive client care
- CPR Certificate
- First Aid Certificate
- Non-violent Crisis Intervention (NVCI) Certificate
- Valid driver's licence and reliable transportation
- Ability to work under pressure and maintain a high level of professionalism
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility and adaptability in a fast-paced environment
- Experience working with individuals with acquired brain injury
- Knowledge of community resources and services
- Ability to work with electronic medical records
- Proficiency in MS Office and other relevant software
