About the Role

We are seeking a skilled Rehabilitation Support Worker to join our team at Brain Injury Services Muskoka Simcoe. As a key member of our interdisciplinary health team, you will work closely with clients who have acquired brain injury to provide support and guidance in achieving their goals.

Key Responsibilities
  • Develop and implement individualized counseling and intervention programs to assist clients in determining their goals and means of attaining them.
  • Prepare reports and case histories to document client progress and outcomes.
  • Collaborate with healthcare professionals to provide comprehensive care and support to clients.
  • Identify additional or alternative services and provide referrals as needed.
  • Interview clients and assess their problems to develop effective support plans.
Requirements
  • CPR Certificate
  • First Aid Certificate
  • Non-violent Crisis Intervention (NVCI) Certificate
  • Valid driver's licence and own transportation
  • Excellent communication and interpersonal skills
  • Ability to work under pressure and maintain confidentiality
