We are seeking a highly skilled and compassionate Case Manager to join our team at the Canadian Mental Health Association. As a Case Manager, you will play a critical role in supporting adults and youth who are involved in the criminal justice system and struggling with mental health challenges.
Key Responsibilities- Collaborate with individuals to identify strengths and needs, and develop long-term goals and short-term objectives to meet these goals.
- Assist individuals in acquiring skills and connecting with formal and informal support, peer support, professional services, and resources to reduce the likelihood of further conflict with the law and improve quality of life.
- Work directly with court staff to explore possible outcomes through the court process and assist clients in addressing the correlation between their offense and mental health needs.
- Provide support and guidance to individuals and their supports in navigating the court system and accessing community-based services.
- Degree or diploma in health or social services.
- Experience working in a community-based mental health setting, with a focus on supporting individuals involved in the criminal justice system.
- Knowledge of systemic issues and social determinants of health, such as poverty, unemployment, stigma, and isolation.
- Ability to apply recovery principles and empowerment-oriented philosophies and practices in work with consumers.
- Strong communication and interpersonal skills, with the ability to build relationships with individuals, families, and community partners.
This is an office-based position with regular travel to community settings, meetings in clients' homes, and other locations as required. A valid Canadian driver's license and proof of insurance are required.
