Job Title: Chronic Disease Prevention Nurse

Job Summary: We are seeking a skilled Chronic Disease Prevention Nurse to join our team at Vancouver Coastal Health.

About the Role: The successful candidate will provide chronic disease management education and clinical intervention to clients at risk of chronic diseases. The role involves assessing clients, developing and implementing education programs, and collaborating with healthcare teams to ensure quality care.

Responsibilities:

  • Provide chronic disease management education and clinical intervention to clients
  • Assess clients and develop individualized care plans
  • Collaborate with healthcare teams to ensure quality care
  • Participate in education program development and implementation

Requirements:

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
  • Two years of recent related clinical experience in a community and/or outpatient care setting
  • Eligible for accreditation as a Certified Diabetes Educator
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes
