Job Overview
Vancouver Coastal Health is seeking a skilled Nurse Case Manager to join their team at the Robert & Lily Lee Family Community Health Centre in Vancouver, BC. As a vital member of the Low Barrier Home Support team, you will play a key role in managing a select population of clients with chronic illnesses or long-term physical disabilities.

Key Responsibilities:
  • Conduct assessments and develop care plans for clients, utilizing chronic disease management principles.
  • Liaise with interdisciplinary teams, physicians, and community agencies to ensure seamless client care.
  • Collaborate with healthcare professionals to provide education and support on disease management, self-care, and medication adherence.

Requirements:
  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three years' recent clinical nursing experience in acute care with a medical and/or geriatric focus, or an equivalent combination of education, training, and experience.
  • Local area travel requires the use of a personal vehicle; valid BC Driver's License required.

Compensation Package:
  • Estimated annual salary: CAD $54,000 - CAD $70,500 (based on 37 hours/week).
