Nurse Coordinator

We are seeking a skilled Nurse Coordinator to join our team at Alberta Health Services. As a key member of our healthcare team, you will play a critical role in providing high-quality patient care and support.
Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop individualized care plans
- Collaborate with healthcare professionals to coordinate patient care and services
- Provide education and support to patients and their families
- Participate in multidisciplinary teams to develop and implement care plans
- Monitor and evaluate patient outcomes and make adjustments to care plans as needed
- Completion of an accredited nursing education program
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
- Minimum of five years facility-based nursing experience, including geriatric experience
- Minimum of two years' experience in community nursing (home care, supportive living or facility living)
- Knowledge of computer applications such as Microsoft Word, Pathways, RAI Tools, Meditech and e-People, Connect Care
- Active Transition Coordinators with up-to-date certification may be given preference
