Job Summary

We are seeking a skilled Nurse Coordinator to join our team at Alberta Health Services. As a key member of our healthcare team, you will play a critical role in providing high-quality patient care and support.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs and develop individualized care plans
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication
  • Provide education and support to patients and their families to promote health and well-being
  • Participate in multidisciplinary teams to develop and implement care strategies
  • Monitor and evaluate patient outcomes to ensure quality care
Requirements
  • Completion of an accredited nursing education program
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • Minimum of five years facility-based nursing experience, including geriatric experience
  • Minimum of two years' experience in community nursing (home care, supportive living or facility living)
Preferred Qualifications
  • Knowledge of computer applications such as Microsoft Word, Pathways, RAI Tools, Meditech and e-People, Connect Care
  • Active Transition Coordinators with up-to-date certification may be given preference
