The Long-term Care Coordinator (LtCC) is a key member of our clinical team, responsible for coordinating day-to-day operations and ensuring high-quality individual care within designated clinical areas. This role supports the seamless movement of individuals through the healthcare system, facilitating access to appropriate resources and services.
Key Responsibilities- Supervise and mentor staff to ensure clinical excellence and adherence to policies and procedures.
- Develop and implement administrative systems, including policies, procedures, and standards of individual care, to ensure efficient and effective care delivery.
- Collaborate with the interdisciplinary team to identify, coordinate, and utilize resources effectively, as well as plan for staff educational needs.
- Conduct clinical assessments and provide counseling and education to families, promoting a culture of patient-centered care.
