About the Role

The Long-term Care Coordinator plays a vital role in ensuring the seamless delivery of quality individual care within a designated clinical area. This position supports the coordination of day-to-day operations, facilitating the movement of individuals through the healthcare system and accessing appropriate resources.

Key Responsibilities
  • Supervise and mentor staff to ensure clinical decision-making, assessments, and counseling/education of families.
  • Review, analyze, and make recommendations on administrative systems, including policies, procedures, and standards of individual care.
  • Liaise with the interdisciplinary team to identify, coordinate, and utilize resources efficiently, as well as plan for staff educational needs.
  • Contribute to a continuous quality improvement environment, ensuring adequate staffing levels and effective resource allocation.
