About the Role:

CarePartners is seeking a skilled Nurse Specialized in Wound, Ostomy & Continence (NSWOC) to join our team. As a NSWOC, you will provide wound and ostomy consultation for patients in our clinic and community. This role requires a strong desire and commitment to making a difference in the lives of our patients.

Key Responsibilities:

• Use your expertise in wound and ostomy care to promote self-management by patients and lead the interdisciplinary team to achieve optimal patient outcomes.
• Develop and implement individualized care plans for patients with complex wounds and ostomies.
• Collaborate with the healthcare team to ensure seamless care transitions and comprehensive patient care.
• Stay up-to-date with the latest research and evidence-based practices in wound and ostomy care.

Requirements:

• Registered Nurse (RN) diploma
• Current College of Nurses of Ontario (CNO) registration in good standing
• Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending
• Several years of escalating experience managing wounds and ostomies
• Strong interpersonal skills and ability to work collaboratively with the healthcare team
