Job Title: Registered Practical Nurse - Transitional Care Unit

Baycrest Hospital is seeking a skilled Registered Practical Nurse to join our Transitional Care Unit team.

Job Summary:

We are looking for a dedicated and compassionate Registered Practical Nurse to provide high-quality patient care in our Transitional Care Unit. The successful candidate will be responsible for planning, delivering, and evaluating nursing interventions, including medications and treatments, to meet the personal care, activation, recreation, and spiritual needs of our patients.

Key Responsibilities:
  • Plan and deliver nursing interventions: Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide client-centred care: Work collaboratively with the interdisciplinary care team to provide comprehensive care that addresses the physical, emotional, and spiritual needs of our patients.
  • Collaborate with the Registered Nurse: Develop and revise patient care plans in conjunction with the Registered Nurse to ensure seamless care delivery.
Qualifications:
  • Current registration: Must be registered in good standing with the College of Nurses of Ontario.
  • Basic Cardiac Life Support: Must hold a current Basic Cardiac Life Support certificate, Level C - Automated External Device.
  • Psychogeriatric experience: Preference given to candidates with experience in mental health assessment and intervention, as well as working with responsive behaviors.
  • Excellent interpersonal and communication skills: Ability to work effectively with patients, families, and healthcare teams.
  • Basic computer skills: Proficiency in Meditech, Microsoft Word, and Excel.
