About the Role

As a Transition Management Director with Grand River Hospital, you will play a critical role in leading the integration of our two hospitals into a single organization. This exciting opportunity allows you to leverage your expertise in healthcare integration and transition management to drive change and improve patient care.

Job Summary

The successful candidate will be responsible for overseeing the planning, execution, and post-integration management of all assigned transition programs and operations during the hospital merger. This includes developing an integration playbook, managing dependencies, tracking program risks and issues, and making key decisions to ensure a smooth transition.

Key Responsibilities

  • Develop and implement an integration plan, including defining roles and responsibilities, establishing communication protocols, and setting key performance indicators
  • Collaborate with cross-functional teams to identify and mitigate operational and people risks, ensuring a sustainable integrated operational model
  • Lead the development of process improvements, workflow standardization, and design optimization to enhance patient care and staff experience
  • Manage budget allocation, expenditure authorization, and financial reporting to ensure transparency and accountability
  • Foster a culture of innovation, embracing emerging trends, research, and evidence-based practices to drive quality improvement and patient satisfaction

Requirements

  • A minimum of 2-5 years of experience leading a hospital human resources department or equivalent role
  • Minimum of 5-10 years of experience in healthcare, with demonstrated experience in formal/informal leadership roles, and a designation in Human Resources from a professional regulatory body or association (e.g., HRPA, CHRL)
  • Demonstrated knowledge of performance measurement, quality improvement, and risk management; ability to identify risks, intervene effectively, and escalate appropriately
  • Excellent consultation, facilitation, negotiation, problem-solving, consensus building/team building, and collaboration skills, with the ability to gain timely consensus and alignment of multiple impacted and interested parties
  • Effectively communicate complex ideas to drive action, accountability, and consensus among teams
