Job Title: Nurse Practitioner

At Brightshores, we are seeking a highly skilled and dedicated Nurse Practitioner to join our team in the Grey Bruce Diabetes Program. As a key member of our interdisciplinary healthcare team, you will play a vital role in providing advanced practice nursing level care to patients at risk for or coping with diabetes and related chronic illness.

Key Responsibilities:
  • Provide comprehensive health care to patients, including health assessments, diagnosis, and treatment planning.
  • Collaborate with the program team and other care providers to deliver care at various sites within Grey Bruce.
  • Develop and evaluate services related to diabetes management and education.
  • Participate in the development and implementation of clinical leadership within the team and to the broader healthcare community.
Requirements:
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (EC).
  • Completion of the Nurse Practitioner Program, Primary Care or Adult.
  • A minimum of 2 years related experience with chronic disease care, with preference for recent experience in diabetes care.
  • Certified Diabetes Educator or willingness to obtain within 1 year of employment and maintain certification.
  • Member of the Diabetes Educator Section of Canadian Diabetes Association.
  • Sound clinical background and strong health assessment and diagnostic skills.
  • Demonstrated ability to work collaboratively as part of an interdisciplinary healthcare team.
  • Excellent interpersonal, communication, and leadership skills.
  • Excellent organizational and critical thinking skills.
  • Self-directed with demonstrated creativity, flexibility, and initiative skills.
  • Demonstrated experience in collaborative development of clinical service programs.
  • Knowledge and experience utilizing the principles of adult learning and chronic disease self-management.
  • Experience working within/or awareness of Aboriginal culture.
  • Valid Driver's license and access to a reliable insurable vehicle.
  • Ability to travel to venues outside of assigned site.
  • Availability to work evenings and weekends to accommodate patient and program needs.
  • Recent satisfactory attendance and performance records.

  • Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores through patient safety knowledge, skills, and attitudes.
