Reporting to the Program Manager, the Grey Bruce Diabetes Program (GBDP) The Nurse Practitioner, as an Advanced Practice Nurse (APN) will strive to provide advanced practice nursing level care to patients at risk for or coping with diabetes and related chronic illness according to Diabetes Canada Clinical Practice Guidelines for the Prevention and Management of Diabetes in Canada 2018 and the Standards for Diabetes Education in Canada (2014).
Care is delivered at various sites within Grey Bruce, in collaboration with the program team and other care providers in the community as well as in acute care environments. Specific target populations include indigenous people, and those without access to primary care.
It is expected that the applicant will meet the standards of practice for NPs in Ontario as outlined by the College of Nurses of Ontario (2020). The NP will collaborate with physician colleagues as necessary to support comprehensive health care. The NP will function in a collaborative partnership with the Diabetes Program Medical Director according to her scope of practice and the organizational policies governing NPs at Brightshores Health System.
As the clinical leader for the program the NP participates in the development and evaluation of services related to her role as well as providing clinical leadership within the team and to the broader health care community in order to advance best practices in diabetes management. This responsibility includes the NP Led Diabetic Foot Ulcer Clinic
Required:
Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (EC) Completion of the Nurse Practitioner Program, Primary Care or Adult A minimum of 2 years related experience with chronic disease care. Preference for recent experience in diabetes care. Certified Diabetes Educator or willingness to obtain within 1 year of employment and maintain certification. Member of the Diabetes Educator Section of Canadian Diabetes Association Sound clinical background and strong health assessment and diagnostic skills. Demonstrated ability to work collaboratively as part of an interdisciplinary health care team. Excellent interpersonal, communication and leadership skills. Excellent organizational and critical thinking skills Self-directed with demonstrated creativity, flexibility and initiative skills Demonstrated experience in collaborative development of clinical service programs. Knowledge and experience utilizing the principles of adult learning and chronic disease self-management. Experience working within/or awareness of Aboriginal culture. Valid Driver’s license and access to a reliable insurable vehicle Ability to travel to venues outside of assigned site Availability to work evenings and weekends to accommodate patient and program needs Recent satisfactory attendance and performance records Adherence to Brightshores ‘CARE’ Behaviours (Collaboration, Accountability, Respect, Excellence) and ‘LEAD’ Competencies (Lead by example, Empower, Achieve results, Develop others) and a commitment to a safe workplace free of violence and harassment Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores through patient safety knowledge, skills and attitudes-
