Job Title: Nurse Practitioner

Job Summary:

The Nurse Practitioner will provide advanced practice nursing level care to patients at risk for or coping with diabetes and related chronic illness. The successful candidate will work collaboratively with the program team and other care providers in the community to deliver care at various sites within Grey Bruce.

Key Responsibilities:

  • Provide advanced practice nursing level care to patients with diabetes and related chronic illness
  • Collaborate with the program team and other care providers to deliver care at various sites within Grey Bruce
  • Develop and evaluate services related to diabetes management and education
  • Participate in the development and evaluation of services related to diabetes management and education
  • Provide clinical leadership within the team and to the broader healthcare community

Requirements:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (EC)
  • Completion of the Nurse Practitioner Program, Primary Care or Adult
  • A minimum of 2 years related experience with chronic disease care
  • Certified Diabetes Educator or willingness to obtain within 1 year of employment and maintain certification
  • Member of the Diabetes Educator Section of Canadian Diabetes Association
  • Sound clinical background and strong health assessment and diagnostic skills
  • Demonstrated ability to work collaboratively as part of an interdisciplinary healthcare team
  • Excellent interpersonal, communication and leadership skills
  • Excellent organizational and critical thinking skills
  • Self-directed with demonstrated creativity, flexibility and initiative skills
  • Demonstrated experience in collaborative development of clinical service programs
  • Knowledge and experience utilizing the principles of adult learning and chronic disease self-management
  • Experience working within/or awareness of Aboriginal culture
  • Valid Driver's license and access to a reliable insurable vehicle
  • Ability to travel to venues outside of assigned site
  • Availability to work evenings and weekends to accommodate patient and program needs
  • Recent satisfactory attendance and performance records
  • Adherence to Brightshores 'CARE' Behaviours (Collaboration, Accountability, Respect, Excellence) and 'LEAD' Competencies (Lead by example, Empower, Achieve results, Develop others)
  • Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores
