Case Manager

About the Role
The Case Manager will work directly with the Director of Health and Social Development to provide assistance to First Nations communities without a Jordan's Principle agreement. This role involves working with families to ensure resources are utilized and addressing gaps in services.
Key Responsibilities
- Deliver case management and outreach services, including service referrals and supporting families in self-advocacy skills.
- Collaborate with community members, internal staff, and external professionals to identify services in the area.
- Maintain an updated inventory of services available for each community for children and youth.
- Track data on children and youth accessing Jordan's Principle Respite services.
- Utilize strong communication skills to ensure no gaps in services to clients and foster collaboration to meet families' needs.
- Apply a critical thinking, strength-based, trauma-informed approach when making choices that impact families, being mindful of privilege and power.
- Protect the integrity and work culture of Dene Nation.
- Perform other duties as assigned.
Qualifications
An undergraduate degree or diploma in a Health & Social Services related field (e.g., family, human and/or social services, child and youth care, educational or behavioral intervention) or equivalent education and experience is required.
